Medication kept in clinic:
Any over the counter medication (Tylenol, Ibuprofen, Midol, etc) to be administered at school should be brought in a new unopened container by the parent or student with the completed and signed Medication Authorization form. This form has been updated and is now 2 pages. If you have more than one medication that you would like your student to have in the clinic you can put additional medications on the 2nd sheet. Prescription medication must be in its original prescription bottle with the original label attached and brought in by the parent, with a completed form and any doctor instructions. If your student will be carrying medication with them that has been prescribed by their doctor (Epi-Pen, Insulin, etc), please have your doctor complete the appropriate health care plan. In addition to the health care plan, parents are to fill out the Self Carry/Administration Form and return with the health care plan to the clinic. .
Individual Health Care Plans
If your child has a medical condition such as asthma, diabetes, seizures, or severe allergies (that may require epinephrine administration), medical forms must be completed by their primary care provider. If they carry any medication and self-administer the medication here at school (or keep it in their backpack, etc) their care provider must state that on the form. Links to each form are below.

Immunizations/Vaccines
Transfer students must show immunization documentation when registering at BHS. Parents of transfer students are welcomed to visit the clinic for clarification of any health concerns or policies that may be different from their previous school district. Current students, please bring in updated immunization records whenever a new immunization is given.
Screenings
Screening for vision, hearing are mandated by VDOE for all 10th graders. If you would prefer that your student, not be screened, you may complete the opt out form and return it to the school nurse.
